My Sd card started showing empty folders in file manager but if i boot into SafeMode it works fine.
It doesn't read on PC.
I couldn't run chkdsk on pc cos it's not been read.
I've tried formatting it also to no avail.
It's a 16GB memory card.

Have you tried connecting the phone to PC while in Safe Mode to see if the PC can detect it?

Pc reads the phone even when not in safe mode but there's no way i can format SD card while it's in phone. Only via card reader and it's not detected while in card reader.
If phone is connected via cable, i only get to transfer files; it shows empty folders on pc also.

Might be a virus or hardware issue with the sd card.
When on PC, check the SD card's properties. Does the Used - Free space stats indicate that there are some files on the storage space?

Yes it does indicate, there's free space but the removable drive icon immediately disappears.

Leave it connected to pc and RUN CMD, type
1. diskpart
2. Listdisk
3. Select disk # (might be 1 or 2, it depend, might the last disk listed)
4. Clean

Do you mean it disappears under My Computer (This PC) or under the system tray?
Also, with the phone conected (and sd card inside), launch Device Manager. What do you have under portable devices
